RimWorld Consciousness: Unconscious, Downed, or Dead?
PAWN HEALTH · RIMWORLD 1.6
PC 1.6.4871 rev590 · Unmodded human colonists unless noted · Updated September 22, 2026
Below 30% consciousness, a normal colonist goes unconscious. At exactly 30%, they can still be awake. At 0%, consciousness loss is fatal unless something prevents death. Pain shock is separate: a typical colonist drops at 80% pain even when consciousness sits well above 30%.

What consciousness actually changes
Consciousness is the pawn's capacity to stay aware and keep working. Open Health → Overview and read it next to the conditions on the Health tab. Mood, rest, and psychic sensitivity are different stats. A happy colonist can still have a wrecked brain; a sleepy one can show normal consciousness.
Drop below the threshold and Moving, Manipulation, Talking, and Eating all hit zero. Breathing and blood circulation keep going on their own. That is why a sedated patient can stay alive in a hospital bed. Above the threshold, consciousness feeds those capacities and several jobs, but each link works a little differently.
| Activity or capacity | Effect of consciousness |
|---|---|
| Moving | The consciousness contribution stops improving at 100%. Damaged legs and direct Moving modifiers still matter. |
| Manipulation | Uses the full consciousness value, including values above 100%. Arm and hand efficiency still matter. |
| Reading | Consciousness directly scales reading speed; sight also contributes. |
| Hacking | The consciousness factor is capped at 120%. |
| Shooting and melee | Consciousness affects Manipulation. The shooting Manipulation contribution caps at 100%; the melee-hit contribution caps at 150%. A larger consciousness number is not an equal percentage increase in hit chance. |
Mechanoids call the same capacity Data processing. Anomaly entities call it Sentience. Odyssey drones call it Performance. Special race rules, deactivation, and death-preventing effects mean a human-colonist read does not transfer cleanly to every creature.
Why did my colonist collapse—or die?
Start with whatever needs action first. Several small penalties can stack into unconsciousness; the last drug or illness that showed up is often only part of the story.
- Stop ongoing harm. Rescue the pawn, tend bleeding, and pull them out of extreme heat, cold, or vacuum. Stopping the bleed does not instantly refill blood already lost.
- Inspect the brain and vital organs. Brain injuries cut the starting value. Blood Pumping, Breathing, and Blood Filtration also feed consciousness. Full failure of a required vital capacity can kill on its own.
- Read current pain. High pain lowers consciousness and can trigger pain shock separately. Use the Health value after painkillers and traits.
- Look for offsets and caps. Blood loss, smokeleaf, disease, alcohol, and anesthetic can sit on the same pawn. A 10% cap is a ceiling, not a floor you get for free.
- Check other causes of death. Lethal disease severity, destroyed vital parts, and other damage rules can kill even when consciousness would stay above zero.
After surgery: 1% can be expected
Sedated anesthetic caps consciousness at 1% and clears pain. That means unconscious, not automatically dead. The cap rises to 70% while woozy and 90% while wearing off. Other injuries or drugs can still leave the real value under those ceilings.
A standard full dose starts at severity 1 and loses 0.8 severity per day. The sedated stage ends after about 6 in-game hours, just below severity 0.8. That is not a promise they stand up then—brain damage, blood loss, pain, or another sedating condition can keep them down. The anesthetic condition can outlast its knockout stage.
Blood loss: the dangerous step is at 60%
| Blood loss severity | Stage | Consciousness effect |
|---|---|---|
| Below 15% | Early blood loss | No direct consciousness offset yet |
| 15% to below 30% | Minor | −10 percentage points |
| 30% to below 45% | Moderate | −20 points |
| 45% to below 60% | Severe | −40 points |
| 60% to below 100% | Extreme | −40 points, then a 10% ceiling |
| 100% | Lethal | Fatal blood-loss severity for an ordinary pawn |
Read the current row only. Stages replace each other; extreme blood loss does not stack every earlier penalty. The ceiling also cannot save a pawn whose other penalties already push consciousness to zero. See tending and bleeding control for next steps.
Plague has two extreme stages
At 80% disease severity, plague adds 60 percentage points of pain and subtracts 30 points of consciousness. At 90%, pain jumps to +85 points, the consciousness offset stays −30, and Breathing takes a −15-point offset. Both stages still use the extreme label.
With an intact brain, otherwise normal organs, and no other modifiers, the later stage lands around 35% consciousness. An ordinary pawn still goes down from the 85% pain. Give that pawn a brain at 70% efficiency and consciousness falls to about 6%. Add moderate blood loss and it hits zero. Plague also kills at 100% severity on its own—consciousness is not the only death path.
Pain shock is a different threshold
Normal pain-shock threshold is 80%. Wimp drops it to 30%. A pawn at 80% pain with an intact brain and no other penalties still has about 69% consciousness: downed by pain, not knocked out by the capacity.
A painstopper removes pain, so it blocks this route into pain shock and the pain bite on consciousness. It does not heal the brain, refill blood, or cancel a consciousness cap. A war mask raises the pain-shock threshold by 10 points and a veil by 5; neither raises consciousness directly.
Pain at or below 10% adds no consciousness penalty. Above that, the penalty climbs toward a max of 40 points at 100% pain. Tiny calculated penalties under one point are ignored, so the real start sits around 12.25% pain. At 50% pain the penalty is about 17.8 points; at 80%, about 31.1.
Can a downed colonist crawl?
Yes, when they are humanlike, awake, at least 8 biological years old, have at least 15% Manipulation, and nothing blocks crawling. Those are eligibility checks, not a guarantee they crawl right away. A pain-shocked colonist can qualify. An ordinary colonist below 30% consciousness cannot: they cannot stay awake, and Manipulation drops to zero. Animals and younger children fail the humanlike or age checks.
Brain scars and joywires shrink the starting value
A standard human brain has 10 hit points. Without special effects, a brain at 6/10 HP supplies 60% efficiency before pain, organ factors, and other consciousness modifiers. A permanent brain scar leaves less room for illness or recreational drugs. Ordinary tending does not erase a permanent scar.
A joywire subtracts 20 percentage points of brain efficiency. On an otherwise healthy brain, that starts you at 80%. It is not a hard 80% consciousness ceiling—later bonuses can raise the final value, and further injuries can lower it. A brain at 6/10 HP with a joywire starts at 40% before pain.
Trauma savant is the odd case: the affected brain ignores missing HP for part efficiency. It also adds 50 points of Manipulation and caps Talking and Hearing at zero. It does not protect against blood loss, drug penalties, or other low-consciousness causes. Read the trauma savant breakdown before treating it as a general recovery plan.
Raise the number by treating the cause
Temporary injury: protect the pawn and let healing run. Ongoing blood loss: stop the bleed and wait for recovery. Heatstroke or hypothermia: fix the environment; severe temperature can put a ceiling on consciousness. Illness: keep treatment up and watch immunity as well as severity. A stimulant can change the number for a while, but it will not repair a scar or beat a lower cap.
Permanent brain damage needs something that can heal permanent injuries. Luciferium can eventually clear permanent wounds, but it locks you into a supply loop and is not an instant rescue. Check luciferium supply and use before committing a pawn to it.
Drugs: offsets, multipliers and caps
−30 points subtracts 30 from the percentage. ×0.65 multiplies the running value by 0.65. Maximum 10% applies a ceiling. Mix them and each one behaves differently.
| Drug or implant | Consciousness effect | What to remember |
|---|---|---|
| −30 points | Also −10 points Moving and −20 points pain. Less pain can partly offset the consciousness loss, but a vulnerable pawn can still die. | |
| +20 points | Also +50 points Moving and pain ×0.1. Addiction and overdose stay separate problems. | |
| +10 points | A temporary stimulant effect, not brain repair. | |
| +10 points | Also changes organs and pain. Unmet luciferium need can impose an 80% consciousness ceiling. | |
| Tipsy ×0.90; drunk ×0.65; hammered ×0.50 | Blackout uses a 10% ceiling instead. Alcohol also changes pain and other capacities. | |
| −20 points brain efficiency | Applied to the brain before the later consciousness modifiers. | |
| Removes pain | No direct consciousness bonus; benefit depends on how much pain it removes. |

Does going above 100% help?
Vanilla consciousness has no universal 100% ceiling. For a pain-free pawn with healthy organs, go-juice, wake-up, and luciferium stack to 100 + 20 + 10 + 10 = 140%. That is arithmetic, not a recommendation to stack drugs.
Higher consciousness can still improve Manipulation and reading. Its Moving contribution is already capped. Go-juice can raise movement anyway because it adds to Moving directly—keep that separate from the consciousness contribution. Mods can rewrite capacities and implants; a mod chip limit is not a vanilla rule.
DLC effects that change the diagnosis
| Expansion | Effect | Meaning |
|---|---|---|
| Ideology | Neural supercharge: +10 points | Temporary bonus. Biosculpting sickness multiplies consciousness by 0.8 instead. |
| Biotech | Pollution stimulus: +5 points; nearby psychic bond: +15 | All pollution-stimulus stages share the same consciousness bonus. Separated bond is −25 points instead of +15. |
| Biotech | Deathless and regeneration coma | Deathless can block ordinary death while the brain survives. Regeneration coma caps consciousness at 10%; its recovery timer pauses while fatal conditions remain. |
| Biotech | Never sleep | Removes the Rest need. It does not add the −15-point consciousness penalty some older guides claim. |
| Anomaly | Mind-numb serum: −5 points | Mental-break protection costs consciousness. Death-refusal sickness uses ×0.95. |
| Odyssey | Extreme vacuum exposure: maximum 10% | Starts at 85% exposure severity; vacuum exposure itself is lethal at severity 1. Restore safe pressure promptly. |
| Odyssey | Psilocap: ×0.90 minor; ×0.70 moderate; 35% cap extreme | Extreme starts at severity 0.85. A 35% cap alone does not cross the 30% unconsciousness threshold. |
The 140% Core example can reach 170% with neural supercharge, pollution stimulus, and a nearby psychic bond. Treat that as one specific stack, not a universal max across every creature and expansion effect. Deathless also will not save you after the brain is destroyed.
How the consciousness calculation works
Start from brain efficiency. Subtract the pain penalty, then apply the Blood Pumping, Breathing, and Blood Filtration factors. If that result is positive, add consciousness offsets, multiply the post-factors, and apply the lowest ceiling. Clamp at zero and round to the nearest percentage point.
× (0.8 + 0.2 × min(Blood Pumping, 1))
× (0.8 + 0.2 × min(Breathing, 1))
× (0.9 + 0.1 × min(Blood Filtration, 1))
If Base > 0:
Consciousness = round(max(0, min((Base + offsets) × post-factors, lowest cap)))
Values in this formula use 1 for 100%. If the base is zero or negative, later offsets are skipped. A consciousness bonus cannot restore a destroyed brain. Caps never raise the value: a 10% ceiling on zero is still zero.
Worked example: smokeleaf plus alcohol
Healthy pawn, no pain, smokeleaf high, drunk alcohol stage. Start at 100%, subtract 30 for smokeleaf, then multiply by 0.65: (100 − 30) × 0.65 = 45.5%, displayed as 46%. Alcohol before the offset would give 35%—wrong order.
Check a colonist's consciousness
Use current Health-tab pain and organ capacities after drug effects. The drug switches below add only their direct consciousness effects, so they do not apply pain relief a second time. This models a standard human brain with ordinary rounding. It is not a full survival simulator.
Moving and Manipulation results show the contribution from consciousness only, assuming normal limbs. They omit direct Moving/Manipulation modifiers, including smokeleaf and go-juice. Deathless, trauma savant, special races, lethal damage, disease severity and mods require separate checks.
Frequently asked questions
Does 30% consciousness knock a pawn out?
No. Exactly 30% still allows an ordinary pawn to be awake. Below 30% is the unconscious range. Other conditions can still cause downing.
Is 1% consciousness always safe?
No. It is above the consciousness death threshold, but the pawn is unconscious and may still die from disease severity, blood loss, vital-organ failure or injury.
Will go-juice wake a sedated pawn?
Its consciousness bonus does not remove anesthetic’s 1% ceiling. Treat the condition imposing the limit rather than assuming a bonus overrides it.
Can smokeleaf kill a colonist?
Yes. Its −30-point consciousness offset can push a vulnerable pawn to zero. Its pain reduction may help the calculation, but is not a guarantee of safety.
Does more consciousness always mean faster movement?
No. The consciousness factor in Moving stops at 100%. Direct movement modifiers, such as go-juice’s Moving bonus, can still make a difference.
