How to cure gut worms
and keep your colony fed
Keep treating the infected pawn until total tend quality reaches 300%. Check the Health tooltip for the next treatment time, and plan for extra food while they recover.
Gut worms do not worsen toward a lethal stage, build immunity, or spread between colonists. Extra hunger, pain, and vomiting can still strain a small colony.
The treatment plan
Start with your doctor and the Health tab
- Inspect the condition. Select the pawn, open Health, and hover over gut worms. While treatment is active, the tooltip shows accumulated quality and the next available tend.
- Assign a capable doctor. Check the patient's medical-care setting and make sure the doctor can reach the patient and permitted medicine.
- Tend when available. The first repeat treatment unlocks after about 48 in-game hours. Follow the tooltip for later tends; remaining treatment time can carry forward.
- Keep adding quality. Three actual 100% treatments are enough. Five actual 60% treatments also work. Lower results mean more attempts.
- Support normal work when practical. Constant bed rest does not build an immunity-based cure. Use a hospital bed for better tending if available, and let the pawn work when their overall health allows it.

Which medicine should you use?
Industrial medicine can shorten an outbreak if it produces better treatments. Herbal medicine is a workable budget option. You can also tend without medicine, but its lower potency usually makes recovery slower.

These are minimum counts, not typical outcomes. Five herbal treatments only finish the job if their total reaches 300%.
A hospital bed adds 10 percentage points to base tend quality; an attached vitals monitor adds another 7. These bonuses help reach the medicine cap, but do not raise it. Glitterworld can help a weaker doctor, although scarce supplies may be more useful for urgent illnesses or surgery.

Protect the food reserve
Feed the patients. Keep the kitchen clean.
Gut worms add +100% to the hunger rate factor. For an otherwise ordinary adult, that roughly doubles food use. Other hunger modifiers can change the final amount, so check the pawn's needs rather than doubling every colony food bill.
Make a practical food buffer
If you use 1.6 nutrition per adult per day as a planning baseline, budget roughly another 1.6 for each infected adult. That is about two extra simple-meal equivalents per day at 0.9 nutrition each. This is a rough reserve, not an exact meal count; vomiting and wasted nutrition can increase consumption.
Use existing food, hunting, or trading for an immediate shortage. Growing rice helps later, but does not feed a hungry pawn today.
Manage vomit and work
Vomiting occurs at a mean interval of one day, not on a daily schedule. Clean kitchen filth promptly. A dirty kitchen can add food poisoning to an already difficult situation.
Keep a competent cook working. Reassign an infected cook when a suitable substitute is available and their symptoms are disrupting meals. No contagion quarantine is needed.

The condition also adds pain and a −5 sick mood effect. Recreation, comfortable surroundings, and meeting ordinary needs help while treatment runs its course. Other injuries or illnesses may still justify bed rest.

DLC options require their respective expansions. Penoxycyline does not prevent gut worms.
When recovery seems stuck
Why won't my doctor tend again?
Check the next-tend timer first. If treatment is available, check Doctor work priority, medical-care settings, access to the patient, and access to allowed medicine. “No medical care” blocks treatment; “doctor care, but no medicine” still permits it.
Do gut worms go away by themselves?
No. Ordinary waiting and immunity gain do not cure them. Keep accumulating treatment quality or use an applicable alternative cure.
Why are they still sick after five treatments?
Count quality, not visits. Five 40% treatments only total 200%. You still need another 100%. If you have accumulated 140% and continue using herbal medicine, the remaining 160% needs at least three more tends.
Can gut worms kill a pawn?
They have no directly lethal progression. Starvation and the consequences of other health problems can still be dangerous, especially if vomiting interrupts meals.
Are they contagious or caused by dirty food?
Gut-worm disease events can affect multiple colonists at once, but the disease does not spread between them. Kitchen hygiene helps control food poisoning; it does not prevent the gut-worm incident.
Do I need a hospital?
No. A doctor can treat gut worms without a hospital or medicine. A hospital bed and attached vitals monitor improve base tend quality, but the cure still depends on reaching 300% total.
Is the treatment the same for muscle parasites?
Both use the same 300% cumulative-quality cure and treatment timing. Muscle parasites reduce moving and manipulation and increase rest needs; gut worms increase hunger and cause vomiting. See the muscle parasites guide.
